Notice of Privacy Practices
Who We Are
AnthologyRX is a telehealth platform that connects patients with licensed independent physicians for the evaluation and management of longevity, metabolic, and performance health protocols. We are a covered entity under the Health Insurance Portability and Accountability Act (HIPAA) and are required by law to maintain the privacy of your Protected Health Information (PHI), provide you with this notice of our privacy practices, and abide by the terms of this notice.
What Information We Collect
When you use AnthologyRX, we may collect the following types of Protected Health Information:
- Health history provided through the eligibility assessment (symptoms, medical conditions, current medications, allergies)
- Biometric information (height, weight, BMI)
- Demographic information (age, biological sex, state of residence)
- Provider communications and clinical notes generated during your care
- Prescription records and pharmacy fulfillment information
- Lab results, if ordered as part of your protocol
- Payment information associated with your subscription
How We Use Your Information
We use your PHI for three primary purposes permitted under HIPAA without requiring your separate authorization:
Treatment. We share your health information with the licensed physicians in our clinical partner network (Bask Health) who evaluate your assessment and, if appropriate, issue a prescription. We also share necessary information with our licensed 503B compounding pharmacy to fulfill your prescription and with any specialist or provider involved in your care.
Payment. We use your information to process your subscription, verify insurance or FSA/HSA benefit claims where applicable, and manage billing.
Healthcare Operations. We use de-identified or aggregate health information to improve our platform, evaluate clinical protocols, ensure quality of care, and conduct internal compliance audits. We do not sell your PHI.
Who We Share Your Information With
We share your PHI only with parties who have entered into a HIPAA-compliant Business Associate Agreement (BAA) with AnthologyRX, including:
- Bask Health — our clinical partner whose licensed provider network evaluates your assessment and issues prescriptions
- Licensed 503B Compounding Pharmacy — to fulfill any prescribed medication
- Payment Processor — limited to billing information necessary to process your subscription
We do not sell, rent, or share your PHI with advertisers, data brokers, or any third party for marketing purposes.
Your Rights
You have the following rights regarding your Protected Health Information:
- Right to Access. You may request a copy of your health records. We will provide access within 30 days.
- Right to Amend. You may request that we correct inaccurate or incomplete information in your records.
- Right to Restrict. You may request that we limit how we use or share your PHI, though we are not always required to agree.
- Right to an Accounting of Disclosures. You may request a list of instances where we have shared your PHI outside of routine treatment, payment, and operations.
- Right to a Paper Copy. You may request a printed copy of this notice at any time, even if you agreed to receive it electronically.
- Right to Withdraw Consent. You may withdraw consent to telehealth services at any time by contacting us at michael@anthologyrx.com. Withdrawal does not affect information already used or disclosed.

How We Protect Your Information
AnthologyRX implements administrative, physical, and technical safeguards to protect your PHI from unauthorized access, use, or disclosure. All data in transit is encrypted using TLS. Access to PHI is limited to personnel and partners who require it to deliver your care.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with AnthologyRX or directly with the U.S. Department of Health and Human Services, Office for Civil Rights:

We will not retaliate against you for filing a complaint.
Changes to This Notice
AnthologyRX reserves the right to change this notice at any time. Any revised notice will apply to all PHI we maintain. We will post the updated notice on this page with a new effective date. If material changes are made, we will notify you by email at the address associated with your account.
